The Glengarry Transitional Care Unit offers specialized support for individuals facing complex challenges related to mental health, physical health, behaviour and substance use. We provide transitional care and assessments for patients to determine appropriate service levels required when transitioning to community.
Staff at the Glengarry Transitional Care Unit work with patients and families to develop discharge plans tailored to their long-term needs and goals. We help transition patients to appropriate community-based locations for continued care and support.
Any resident of the Island Health region may be eligible. Length of stay depends on individual need.
