COVID-19 Virtual Monitoring

The COVID-19 Virtual Monitoring program provides monitoring and symptom support to people with mildto moderate symptoms of COVID-19.

Once enrolled in the program, you complete a series of questions designed to help the monitoring nurses evaluate your symptoms and monitor you remotely. Any monitoring equipment is provided to you at no cost.

Your monitoring nurses receive alerts as you enter information and communicates with your family doctor or nurse practitioner as required. Nurses are available 7 days a week, from 8 a.m. to 8 p.m.

Monitoring continues until your symptoms are lessened or gone. The monitoring nurses will help you to determine when it is time to come off monitoring and other Community Health Services may be recommended to you.

The program is suitable for people who are immunocompromised or diagnosed with a chronic disease known for poor outcomes when paired with a diagnosis of influenza, such as heart disease, diabetes, lung disease, hypertension, chronic kidney disease, chronic liver disease, or cerebrovascular disease.

What’s Involved in Monitoring

Participation in the monitoring program only takes a few minutes each day. Whether participating online or by phone, you will answer questions about your health including:

  • How you are self-isolating
  • Your temperature
  • Using medicine for fever
  • Your symptoms

Discharge from the program will be arranged once the individual completes their isolation period, at which time they may want to be transferred to another remote patient monitoring service. Learn how to transfer to Community Health Services.

COVID-19 Virtual Care Monitoring Program

The Community Virtual Care Program provides COVID-19 monitoring support for adults with COVID-19 symptoms who require a greater level of support to manage their symptoms at home. Referrals can be made by anyone (including the patient) by contacting Community Health Services

What's Involved

Participants enrolled in the service complete a daily monitoring plan that includes reporting their symptoms, temperature and oxygen levels to a registered nurse throughout the day. The nurses are available by phone 7 days a week from 8 a.m. to 8 p.m. to answer questions or communicate significant changes to a Primary Care Provider (family physician or nurse practitioner).
