Warfarin Initiation Clinic
The Warfarin Initiation Clinic utilizes a nurse-only model that makes use of a Clinic Order Set (COS). The COS is to be completed as part of the referral process into the clinic.
The Warfarin Initiation Clinic provides care and management of diagnosed Deep Vein Thrombosis, Pulmonary Embolism, Atrial Fibrillation, prosthetic heart valves and other conditions which require anticoagulant therapy with Warfarin and Enoxaparin until Warfarin levels are therapeutic.
In order to receive care in the Warfarin Initiation Clinic, patients:
- Need the ability to return to the clinic daily for a minimum of six days
- Cannot be pregnant
- Have no history of Heparin Induced Thrombocytopenia (HIT)

Physician referral required.
